Did You Participate in Refresh From Stress: 30 Day Inspiration?  We Want to Hear From You!

Your feedback about your RFS experience is important to us.  Even if you only participated in one RFS activity or did not complete any, we still encourage you to share your feedback. The results of this survey will allow us to evaluate this program and improve future emotional well-being and stress management offerings.  Only aggregate (group) data will be reported; no individual identifying data will be shared.  The survey will take approximately 5-10 minutes to complete. Those who complete the survey will have an opportunity to enter a drawing for prizes including $20 Amazon gift cards, and $10 Karma Cards from Homegrown Restaurants.

Please provide your feedback no later than Wednesday, November 21.
