Family Medical Leave - FMLA
Eligible employees may take up to 12 workweeks of unpaid, job-protected leave under the Family and Medical Leave Act in a rolling 12-month period for specified family and medical reasons.

Before entering Family Medical Leave information, verify if the employee should be placed on paid or unpaid leave.
- Paid Leave - The employee has unused, accrued time applicable to the type of leave. You need to verify the date at which the applicable accrued time will be exhausted and the employee leave time must be changed to unpaid leave.
- Unpaid Leave - The employee has exhausted all accrued time applicable to the type of leave. These employees also do not accrue time while they are on unpaid leave. They are paid through the short-term disability insurance at 60% pay (if eligible). While on unpaid leave, they also are responsible for and billed for the employee contribution to the benefits deductions.

FMLA and Medical Leave Request Management
- Supervisors/Managers and departmental Human Resources Representatives may review and approve employee requests for FMLA or Medical Leave online. Employee's supervisor/manager and departmental Human Resource Representative are immediately notified via e-mail when a request is submitted. Employees receive e-mail notification when leave has been approved by departments and Human Resources.
- Employees requesting personal, educational or military leave should use the Non Medical Leave Request Form.
