Emory Clinic provides members with basic term life insurance and supplemental life insurance at no cost. Members also have the option to elect voluntary supplemental life insurance coverage. All life insurance benefits are administered by The Standard Insurance Company.

About Life Insurance Coverage

Basic Life Insurance

Emory Clinic provides eligible physician members, professional members, senior/founding members and key executives with basic life insurance in the amount of up to $50,000. There is no cost and enrollment is automatic.

Supplemental Life Insurance

Emory Clinic also provides supplemental life insurance coverage in the amount of $450,000. There is no cost and enrollment is automatic.

Voluntary Supplemental Life Insurance

You can elect voluntary supplemental life insurance coverage in increments of $50,000, up to a maximum guaranteed issue of $250,000 at the time of your initial eligibility without Evidence of Insurability (EOI). Coverage in excess of $250,000 up to $500,000 will require EOI. Premiums will be deducted from your salary monthly. If you initially waived coverage, EOI is required.

Voluntary Supplemental Life Insurance for Spouse

You can elect voluntary supplemental life insurance coverage for your spouse in the amount of $4,000 as a rider on your Supplemental Life Insurance coverage. Additional voluntary supplemental life insurance coverage for your spouse is available in increments of $50,000, up to a maximum guaranteed issue of $50,000. Coverage in excess of $50,000 up to $500,000 will require EOI. Premiums for life insurance coverage on your spouse will be deducted from your monthly salary. If you initially waived coverage, EOI is required.

The amount of dependent life insurance for your spouse may not exceed $100% of the amount of your voluntary supplemental life insurance coverage.

Voluntary Supplemental Life Insurance for Child(ren)

You can elect voluntary supplemental life insurance for your eligible child(ren) in the amount of $4,000 as a rider on your supplemental life insurance coverage. Additional voluntary life insurance coverage for your child(ren) in increments of $10,000, up to a maximum of $30,000.

The amount of dependent life insurance for your spouse may not exceed 100% of the amount of your voluntary supplemental life insurance coverage.

Coverage begins on the date of hire or upon approval of Evidence of Insurability (EOI) if required.

Reduction in Insurance

At age 70, life insurance coverage for you and your spouse reduces to 65% of the original face amount; at age 75 it reduces to 65% of the amount of insurance coverage in effect immediately prior to age 75.

Eligibility

Benefits-eligible members of the Emory Clinic scheduled to work 20 hours or more per week are eligible. You are a member if you are one of the following:

  • Physician Member
  • Professional Member
  • Senior/Founding Member
  • Key Executive

If you enroll, your dependents are also eligible.

Coverage begins on a member's date of hire.

Basic and Supplemental Life Insurance

Emory Clinic provides basic term life insurance and supplemental life insurance so there is no cost to you.

Voluntary Supplemental Life Insurance

Premiums for voluntary supplemental life insurance coverage will be deducted from your paycheck. Rates are listed below.

Below is an example of how the cost for a voluntary supplemental life insurance policy is calculated for a physician member, age 42, who elects a $100,000 voluntary supplemental life insurance policy.

  • Cost = ($100,000/$1,000) x .10 = $10.00 per month

Voluntary Supplemental Life Insurance for Child(ren)

Cost is $0.15 monthly per $1,000 of dependents life insurance per member electing dependent life insurance, regardless of the number of children covered.

How to Enroll

No enrollment is necessary for basic life insurance and supplemental life insurance. This benefit is automatically provided when a member is eligible.

You must enroll for voluntary supplemental life coverage using Self-Service.

Evidence of Insurability

If required, complete the online medical Evidence of Insurability (EOI) form. EOI must be approved by The Standard. Approval can take 6-8 weeks. Emory Clinic's policy number is 649350-A. To inquire on your EOI status, contact The Standard at 866-756-8118.

Designate a Beneficiary

You should designate a beneficiary for each life insurance benefit you are enrolled in.

Your beneficiary is the person(s) who will receive your life insurance benefits when you die. Your beneficiary can be one person or multiple people, charitable institutions or your estate.

Once named, your beneficiary remains on file until you change it. If your family situation changes, review the beneficiaries on file and make updates if necessary.

If you do not name a beneficiary, your life insurance benefits will be paid in equal shares to the first surviving class of the classes below.

  • Your spouse
  • Your children
  • Your parents
  • Your brothers and sisters
  • Your estate
