COST 

Emory Clinic provides basic term life insurance coverage and supplemental life insurance. Premiums for voluntary supplemental life insurance coverage will be deducted from your salary.

  MEMBER VOLUNTARY SUPPLEMENTAL LIFE

Example:

Below is an example of how the cost for a voluntary supplemental life insurance policy is calculated for a physician member, age 42, who elects a $100,000 voluntary supplemental life insurance policy.

Cost = ($100,000/$1,000) x .10 = $10.00 per month

  DEPENDENTS LIFE INSURANCE

Supplemental: $1.50 monthly per member electing dependents life insurance, regardless of the number of dependents covered.

  VOLUNTARY SUPPLEMENTAL LIFE INSURANCE

For a Child: $0.15 monthly per $1,000 of dependents life insurance per member electing dependents life insurance, regardless of the number of children covered.
