Emory offers the opportunity to enroll in Long Term Care Insurance provided by UnumProvident.

Eligibility

Emory offers Faculty, Staff and Post-Doc employees working 20 hours or more a week and are classified as regular full time or part time, the opportunity to enroll in Long Term Care Insurance. Temporary employees are not eligible.

Family members of eligible employees are also eligible for Long Term Care Insurance. This includes a spouse/same-sex domestic partner, adult children, siblings, parents, parents-in-law, grandparents and grandparents-in-law.

How to Enroll

  • Employees:
    • Complete an enrollment form* and send it to UnumProvident.
    • If you are a new hire, you have 31 days from the date of hire to enroll and be assured a Guarantee Issue. This means you do not have to go through medical underwriting. However, if you do not enroll at this time, you must complete a medical questionnaire and be approved by UNUM for coverage. There is no guarantee that coverage will be approved.
    • Premiums are paid through payroll deduction.
  • Spouses, Same-Sex Domestic Partners of Employees:
    • Complete an enrollment form* and a medical questionnaire. Send these to UnumProvident. This must be approved by UNUM. There is no guarantee of coverage.
    • Premiums are paid through payroll deduction.
  • Family members of eligible employees:
    • Contact UnumProvident to request an enrollment form and medical questionnaire. Complete the forms and return them to UnumProvident. This must be approved by UNUM. There is no guarantee of coverage.
    • Premiums are paid directly to UnumProvident.

* For employees, spouses, same-sex domestic partners of employees, if you do not have an enrollment packet, which includes the enrollment form and medical questionnaire, contact the Benefits Department at (404) 727-7613.

Plan Coverage

Benefits are paid when a physician certifies you have lost the ability, for a period of 90 days, to perform two out of six Activities of Daily Living (bathing, dressing, toileting, transferring, continence, and eating), or you suffer severe cognitive loss (like Alzheimer’s Disease).

Once you qualify for long-term care benefits, you receive payments whether receiving care in a nursing home, assisted living facility or utilizing professional home care. You must satisfy a 60-day elimination period before benefits are payable.

Plans Available

  • Plan 1 (Base Plan): Provides a facility monthly benefit that does not change over time.
  • Plan 2 (Base Plan with Compound Inflation Option): The facility monthly benefit chosen at enrollment increases each year by 5% compound. With this plan, the facility benefit, home benefit and lifetime maximum increases with each year of participation.

The younger you are when you enroll, the lower the cost. As long as you remain enrolled, the rates do not change as you get older or use the benefits.

Benefit Levels

Whether you elect Plan 1 (Basic Plan) or Plan 2 (Basic Plan with Compound Inflation Option), you need to decide:

  • Your Facility Monthly Benefit: You can elect $1,000, $2,000, $3,000, $4,000, $5,000*, or $6,000* for your monthly benefit to facility for your care. (*Medical underwriting required.)
  • Your Facility Duration Benefit: You can elect 3 years, 6 years, or unlimited years* for your care at a facility. (*Medical underwriting required.)

Plan Costs

Your cost is determined by:

  • your choice of Plan 1 or Plan 2
  • your Facility Monthly Benefit
  • Your Facility Duration Benefit
  • your age at enrollment

Long Term Care Rates are subject to change in the future depending on the overall use of the benefits of all covered persons or changes in the benefits levels, plan design or other risk factor.
